@michelle g @ANNA F From GD: Can you verify please if the original dust caps were replaced with the old style steel caps and those fell off? If so, then next steps would be to replace all 4 hubs with new to resolve this issue.

Yes, we did replace them with the old style steel caps, the old style steel caps did not fall off, it is the new style that are falling off.
