Submitted for PA. @Brian P, one of the pictures required is: The glass removed showing the springs at the bottom - do not remove any cover over springs for touchscreen.
I didn't see that picture in what was sent over to me, but wanted to make you aware so you could get it. I submitted for PA, but it could be required for payment.

At time of claim submission, will need a photo of the cut cord against the tag showing the model/serial numbers.
Additional details included in DropBox in the "Approval" PDF.
