@Jeannette B Please send the model/serial numbers for line 7. Tech doesn't actually have that he is replacing it..is that what is needing to be done or does he want a recommendation from FR to see if there is something else that should be replaced?

I spoke with the tech and he stated that the water pump got hot and shut off. He is requesting replacement. I have added a photo of the part that shows the model/serial number.

@Jeannette B For line 2 - FR is asking what caused the damage. I had thought maybe that upper piece falling down, but I'm not sure if that would explain all the damage shown in the picture.
For line 6 - FR needs a picture of where the leak is coming from. I'm going to email you a picture that I have selected an area that looks like it is where the leak is coming from - can you just confirm it once I send it to you?
